Fees

The initial intake is the name for the first appointment. During this 53-minute meeting we will conduct a wide ranging interview covering the history of the presenting issue and any other relevant historical information.  The charge for this session is $210. This fee covers the time spent in the interview as well as interpretation of any screening or diagnostic evaluation tools.  The fee also goes toward any consultation with teachers, counselors, or other clinicians if necessary, and review of records, if available.  The fee for subsequent sessions is $180 per 53-minute hour.  I accept most major credit cards for payment. 



 

 

 

​Insurance

I am an in-network provider for Premera, LIfewise, First Choice Health Networks and Kaiser PPO plans.  I am also in-network for most Blue Cross Blue Shield plans, with rare exceptions. The exceptions that I know of are Regence, Blue Cross of Illinois (Boeing), and Blue Cross Blue Shield Federal Employee Plan (FEP).  Since I don't work for any insurance company I cannot garauntee coverage, so if you're in doubt I'd recommend calling the number on the back of your insurance card and ask If Dr. Darryl Graham is in your network.

 

For subscribers of the above plans I can bill on your behalf and all you'll have to pay for at the sessions is a copay and your annual deductible amount if relevant.  For other insurance carriers I can provide subscribers with receipts with all the essential information that can be used by the subscriber to submit your own claims to the insurance provider for reimbursement. I can also help locate claim forms online and assist in filling out the forms.  Please note that it is the responsibility of the subscriber to be aware of the benefits and reimbursement rates of insurance providers.
